Food Handlers Cards - Food handlers classes are available online or at the health department in Logan and Brigham. Tests can be taken in Logan or Brigham please call for an appointment.

Food Safety Manager Certification - Every food establishment must have at least one person with this certification. This is not provided by the Bear River Health Department. Approved providers of this certification are Utah State University, NRFSP, and ServSafe. A copy of this certificate needs to be submitted to the Bear River Health Department. 

Restaurant Inspections - Inspections are conducted twice annually at every food establishment to evaluate the risk of food contamination. Search for inspection results here.

Temporary Food Service Permit - This applies to all temporary food service that is offered to the public.

Mass Gathering - If your gathering lasts more than 2 hours and will be attended by more than 500 people, you must obtain a Temporary Mass Gathering Permit. If your gathering has food service you must also obtain a Temporary Food Service Permit.

New Food Establishment and Information Packet - Is your food establishment new, changed locations or owners? If so please fill out this packet.

Mobile Food Service Requirements - Answers to question about requirements for mobile food establishments.

Foodborne Illness Complaint Form - Reporting illness is an important part of disease tracking. This will help us determine of their has been a breakdown of food safety practices at an establishment. Please return this form to our offices by fax, in person, or call us directly at 435-792-6570. Please seek medical attention if you are currently experiencing symptoms associated with foodborne illness.
