What services are offered?

  • Exams for women including breast and pelvic exams and pap smears are offered on a sliding fee scale
  • Low cost birth control and free condoms for those who qualify
  • Confidential testing a treatment of sexually transmitted infections (STD’s)
  • Health care counseling to prevent disease and improve health
  • Referral to other services as needed

 

Who is this program for?

How do I make an appointment?

  • Call 792-6500 for an appointment. Initial exams will be provided in Logan. Follow up services such as birth control refills can be provided in the Logan office or Brigham office. 
  • When calling for an appointment please let us know if you have had a yearly exam from another provider in the last year. If so you can sign the release of information and get your records to bring with you. Click here for the Records Release Form

 Frequently asked questions

What if I have already had an exam at my Doctors' office?  The health department can do a records visit only. The cost is $15. If your exam has been done in less than a year ago, we can do a written request for records and use these for the exam.

Do you carry other forms of birth control besides the pill?  No. Free condoms are available for those who qualify.

What if I don't need birth control? If you just need an exam, or need your IUD strings checked, we can do a low cost exam including a pap smear.

What if I have insurance? Women with insurance are welcome. We do not bill insurance for this service, but can provide an itemized statement if needed.
