The Bear River Health Department offers low or no cost immunizations for those who qualify. The health department does not charge for an office visit.

Insurance accepted for immunizations:

  • Select Health (Select Care + and Select Med +)
  • Medicare Part B
  • Medicaid 
  • PEHP Advantage
  • PEHP Traditional
  • Blue Cross/Blue Shield
  • EMI Health 
  • Altius

Please check with your insurance to verify immunization coverage. 

If your insurance does not cover immunizations or you are currently uninsured please call the health department at 435-792-6500 in Logan and 435-734-0845 in Brigham.

Clinic Hours

 

Know Before You Go  - When getting immunized it is helpful to provide the following information.

  • Immunization records, this may be available from a physicians office.
  • Insurance card
  • List of immunization covered by the insurance provider and if the health department is a "preferred" or "in network provider". If you are unsure contact your insurance company.
  • Birth date of the person receiving the immunization.
  • A list of any allergies or any reactions to previous shots.
  • Health history, including any medications currently being taken.
